Infant Mortality Self Assessment Matrix - Developed by ChiMat in conjunction with Yorkshire & Humber SHA this tool will enable SHA's and PCT's to evaluate their progress in implementing the plan for reducing health inequalities in infant mortality: a good practice guide.

Maternity Matters Self Assessment Matrix - The questions in this self-assessment are a sub-set of the questions in Standard 11 of the NSF for Children, Young People and Maternity.

The decision support tool will assist you with the thought processes around designing future services across a managed clinical network. A self-assessment tool for commissioners, published in tandem with Maternity Matters, to help PCTs and their partners ensure local maternity services meet population requirements and address health inequalities Strategic commissioning: a state of readiness assessment - maternity services, and children's and young people's services - These working drafts of self assessment tools for PCTs are based on the PCT fitness for purpose programme and have been adapted to highlight key priorities for commissioning services that meet local population requirements and address health inequalities.

Self Assessment Matrices - This website provides access to four tools which are designed to help Commissioners and local health economies assess their progress toward implementation of the the National Service Framework for Children, Young People and Maternity Services.

The Self Assessment tools for the CNSF which include the Child and Maternity Mapping - This is an annual data collection exercise for child health services and child and adolescent mental health services, Its primary purpose is to support the implementation of the child policy agenda in particular the National Service Framework for Children and Young People.
