National service framework for children, young people and maternity services. The Children's NSF, published on 15 September 2004, sets standards for children's health and social services, and the interface of those services with education up to age 19.

These standards include a section on "Growing up and moving on to adult services [4.58 to 4.62]
Getting it Right This Good Practice Guide aims to show that the handover from children's and young people's services to adult services should be planned and managed as a process. The Guide suggests how this can best be accomplished in the context of the evidence base.
